Your Donation, Dive Club & Tax Receipt Information Form
For all donations of $20 or more, Tobermory Health Services Auxiliary will issue an official federal income tax receipt.
Please use this form to supply us with your mailing address for your tax receipt as well as your Donation Recognition preference and if your donation is affiliated with your local Dive Club/Shop.
